Medical Disclaimer

NOT A MEDICAL DEVICE. NO MEDICAL ADVICE. PacketVisit is designed solely for general wellness, personal tracking, and self-reporting. It is not intended to diagnose, treat, cure, or prevent any disease or medical condition. PacketVisit is not a medical device and has not been cleared or approved by the U.S. Food and Drug Administration (FDA), the European Medicines Agency (EMA), or any other regulatory body under the EU Medical Device Regulation (MDR). The visit packet you generate is a self-reported summary and does not constitute an Electronic Health Record (EHR). Always consult a qualified healthcare professional before making medical decisions. In a medical emergency, call local emergency services immediately (e.g. 911 in the US/Canada, 112 in the EU). Information you enter is self-report only.

1. Intended use

PacketVisit helps you (the individual patient / journal user) record experiences in structured lists and export a summary you may choose to share with a licensed clinician. It is a personal B2C wellness tool — not a clinic-operated charting system. Interpretation of that information is solely between you and your clinician.

2. No diagnosis or triage

The app will not infer diagnoses, risk scores, or “likely conditions” from your entries. Export summaries are descriptive tables and counts — not clinical conclusions.

3. Not a substitute for professional medical care

PacketVisit is not a substitute for a licensed clinician, diagnosis, treatment plan, or emergency services. Do not delay or avoid seeking professional medical care because of anything you see in the app or in a visit-packet export.

4. Self-directed use — your responsibility

If you use PacketVisit to track symptoms or decide what to do without consulting a clinician, you do so at your own risk. Health decisions (including medication changes, delaying an appointment, or self-managing pain) remain yours alone. The maker of PacketVisit does not assume clinical responsibility for those decisions or for any health outcome.

5. Chest symptoms — out of scope

PacketVisit is not for chest pain, chest pressure, shortness of breath, or symptoms you worry may involve your heart or lungs. If you have these symptoms, seek medical care now (emergency services if severe). Do not use this diary for chest symptoms.

6. Emergencies

PacketVisit is not monitored by clinical staff. If you think you may be experiencing a medical emergency, call your local emergency number immediately (for example 112 in the European Union, 911 in the United States and Canada). Do not wait for an app export or appointment summary.

7. Medications

You may optionally note medications you have taken in your own words. PacketVisit never recommends drugs, doses, or treatment changes.

8. Weather context

Automatic weather snapshots are environmental context only. They do not prove that weather caused your symptoms, and they are not a medical forecast.

9. No warranty of clinical outcomes

Visit packets and diary entries are tools for your own notes. We do not warrant that exports are complete, accurate for clinical use, or suitable as a substitute for professional records. THE SERVICE IS PROVIDED “AS IS” TO THE MAXIMUM EXTENT PERMITTED BY LAW.

10. Assumption of risk

By using PacketVisit you assume the risks associated with relying on a self-report wellness journal. Limitation of liability is set out in our Terms of Service. Nothing here limits non-waivable consumer rights where they apply.

11. Regulatory position

We position PacketVisit as a general wellness / personal journal product for structured self-report and optional export. It is not intended to be a medical device under frameworks such as the EU Medical Device Regulation (MDR 2017/745) or as an FDA-regulated diagnostic or treatment device in the United States. Regulatory labels differ by region; this paragraph does not constitute a formal conformity assessment. If your clinician asks for clinical systems or EHR integration, that is outside the current product scope.

12. Geographic note (US, Canada, EU)

Product and privacy notices are written for users in the EU/EEA, United Kingdom, United States, and Canada. Local mandatory consumer and health rules may add rights that this disclaimer cannot reduce.

13. Agreement

By using PacketVisit you acknowledge this disclaimer and agree that you will not rely on the app as a substitute for professional medical care.

PacketVisit
